Because of its poor prognosis, CFS is often managed in the primary care setting where a definitive diagnosis is lacking. There is a growing interest in finding and validating new strategies for managing this condition. Recently, the International Association for Chronic Fatigue Syndrome and Immune Dysfunction Syndrome (IAS) has formed a new special interest group in order to explore novel therapies. One particular approach to CFS/FM has been through dietary interventions. It is well documented that individuals with CFS/FM frequently have vitamin D and magnesium deficiencies.

Many experts have suggested that both are required for optimal immune function, and that supplementation is not only beneficial but necessary for optimal health. There is emerging research supporting the use of high dose vitamin D as a treatment for CFS/FM. To date, two small studies have reported a decrease in fatigue scores with high dose vitamin D. However, an increase in energy scores was also noted, making it difficult to know whether vitamin D treatment actually improved fatigue, rather than increasing energy.

In addition, one study reported significant reduction in the levels of a cytokine thought to play a role in CFS/FM, suggesting that vitamin D might decrease the frequency of CFS/FM attacks by altering immune function. Further study in a placebo controlled fashion is needed to clarify the efficacy of high dose vitamin D therapy for CFS/FM. While the benefits of magnesium therapy are not clear, there is ample evidence that magnesium is essential for an appropriate immune response and that both CFS/FM and the flu are associated with low serum levels.

What Research is Taking Place?

More research on the benefits of magnesium therapy for CFS/FM is required. While further studies are needed, preliminary data suggest that a large proportion of the approximately 8 million patients with CFS/FM in the U.S. could benefit from dietary supplementation with both vitamin D and magnesium. These findings and preliminary research suggest that the time may be right to establish high dose vitamin D and magnesium supplementation as a primary therapy for a disease previously considered by many to be ineradicable.
